My two most seen bands by far are Phish (18 times) and of Montreal (12 times). I don't think anyone else is over 5.

I do not maintain a trapper keeper but, here goes my best estimate:
Most: Cheap Trick (at least 20 times)
10+: Paul McCartney, Ringo Starr/All Starr Band, Material Issue, Bob Mould/Sugar
Phish: zero times.
HM - The Cure 9 times

-Against Me! definitely close to 20 on that one
-in order of most: Against Me!, The National, QotSA, Mastodon, Coheed & Cambria, Deftones, i think NIN and Radiohead just hit the 10 mark, maybe Pallbearer and Baroness, Tool and Ghost are pretty close. i'm sure Slayer is in the double digits but i can't be bothered to read all the old mayhem lineups.

Sondre Lerche and MMJ are probably the ones I've seen most.

I've seen Local H 8x. I left Bonnaroo 2012 early to avoid seeing Phish. do Illinoisans get to count local h? even in central IL i saw them like half a dozen times (kidding. this made me realize Chevelle is actually probably my #1)

joe ely (with the flatlanders, with his full band, solo, or with one or two other musicians): 16 or 17 times.
10 + times: the now late Albertina Walker (the Chicago Gospel festival had her there every yr), the house band for WBEZ's flea market (led by either Jim Post or Stuart Rosenberg), the Riccardos (local Wash U. band that included a fraternity brother).
once (opening for Santana at Poplar Creek in 1992, and then joining Carlos during his encore).

Clutch and Coheed. Seen both at least a dozen times each. Seen Primus a bunch, too.

Never seen anyone over 10 times.
Seen CHVRCHES 7 times (will be 9 following this week's aftershow and Lolla) The National / Florence / LCD Soundsystem would probably be next at ~5x each

R.E.M. and Pearl Jam are both technically tied at 8 - I did see the former live during a taping of the Today show where they played 3 songs, but I don't count that.

Can't imagine anyone that I would, honestly. living in a place like brooklyn where young, up and coming east coast bands play a lot for cheap and brands/websites sponsor free shows helps drive up the tally on certain acts. couldn't imagine doing it for bands where their touring shows regularly cost $15-$30 each.

Fair. I admittedly didn't take enough advantage when I was in Chicago. Definitely not something likely in Denver unless I fell in love with a local band.
